Unable to add providers, facilities, patients in new install of FreeMED 0.8.4

Hi,
I'm trying to add data to a new installation of FreeMED 0.8.4, and I after submitting a form I get the response 'Record addition failed'. I couldn't find any text logs (are there any?) and the log table in the database only shows logins. I'm logged in as admin. I *was* able to add a user, but not patients, providers, or facilities.
Is there some initial config step that I missed?
My setup:
FreeMED 0.8.4
Windows XP
Apache 2.2.6
PHP 5.2.5-Win32
MySQL 5.0.45-win32
Everything is running on local machine, FreeMED installed under \Apache2\htdocs\freemed.

First did you install via the instructions on a M$Windows install? Next are you browsing with Internet Explorer, or Firefox? Long story short on IE, their are many glitches and problems with functionality using that browser with FreeMED (especially administration). Try using firefox instead to see if that cures some of your ills. If still no success I would try 1 of 2 things: 1) do a complete reinstall of your server, freemed, and remitt following the above windows install link. However you should realize some of the functionalities of FreeMED are currently hard-coded to work in a LINUX (possibly UNIX) environment. Which means it will take a lot of manual coding and hacking to make it work on windows. Or 2) download something like vmware player and a virtual appliance of debian or ubuntu and install via the linux install instructions. IMHO, the later (although initially intimidating) will be a better solution and considerably more pleasent in the long term.
